Abstract: This research paper investigates the impact of personalized medication counseling on patient adherence and health outcomes in individuals with chronic conditions. Utilizing a retrospective study design, we analyzed data from 200 patients who received either tailored counseling provided by pharmacist or standard care. Results indicated a significant improvement in medication adherence and positive changes in key health indicators, including blood pressure, HbA1c levels, and LDL cholesterol levels, among those receiving personalized counseling. The findings highlight the importance of individualized care in enhancing treatment outcomes for patients with chronic diseases.
