Abstract: This qualitative study investigates the barriers clinical pharmacists encounter when implementing medication monitoring and management strategies in critical care units. Through in-depth interviews, the study explores pharmacists' perspectives on challenges such as medication errors, communication gaps, and resource limitations, offering insights into improving patient safety and healthcare outcomes in intensive care settings.
