Abstract: This study evaluates the outcomes of pharmacist-driven protocols for managing anticoagulation therapy in hospitalized patients at a large tertiary hospital. A retrospective analysis of 500 patients revealed that those managed under pharmacist-led protocols experienced a significantly lower incidence of major bleeding complications (6.0% vs. 11.2%) and had a higher mean time in therapeutic range (TTR) for warfarin therapy (68.5% vs. 55.3%) compared to those receiving standard care. Although the reduction in thromboembolic events was not statistically significant, the findings underscore the critical role of pharmacists in optimizing anticoagulation therapy and enhancing patient safety.
